- Credit Application on file - please note that all orders are shipped on prepaid terms, this
form provides us with your contact, business & shipping info only at this time.
- Credit Card Authorization Form - this form gives us permission to use your credit card for
payment of your order, you simply mark for a one time use [additional orders will need additional
forms] or for all future orders. If you will be order only thru our website, this form does not have to
be completed. For phone/fax/email orders, this form is required.
